ACS Home Care, LLC Assistant Community Liaison in Yonkers, New York

Responsibilities:

Represents company as a front-line customer service liaison between patient/clients, Patient Assistants (PA) and the Administrator. Manage weekly schedules on CDPAP consumers to maintain clock in/out based on EVV. Record and monitor daily attendance of all assigned cases. Must follow strict compliance with agency policy and regulations. Ability to maintain accurate schedules, notes and records. Addressing payroll and human resource issues with PAs. Acting as a liaison between the agency and the community regarding community and customer needs. Establishing and maintaining effective working relationships with staff, the public, and healthcare professionals. CDPAP Coordination: 1 year (Preferred)

Qualifications: CDPAP Coordination - 1 year Proficient in HHA Exchange Computer literate is a must - Microsoft word excel and email. The candidate for this position will have a high school diploma (or equivalent) and 1-3 years related experience in health care delivery or business. Flexible and able to multitask, effective time management skills. Must be organized and detail-oriented with excellent listening, written, and oral communication skills. Self-motivated, able to work independently and with teams, multitask, and make sound decisions when needed. Bilingual Spanish/English. Other languages are a plus
