Wage - $22.00 - $25.00 Schedule - Monday - Friday, 8 am to 5 pm Benefits * Medical/Dental * FSA * 403(b) Retirement * Additional Voluntary Benefits * Paid Holidays * Vacation/Sick Leave * Education Assistance Program * Employee Assistance Program * Longevity awards Position Summary: The ECLIPSE Home Visitor is responsible for home visits and case management of children and families enrolled in the program, participating in assessment, treatment planning and multidisciplinary meetings and connecting children and families with resources and services. Responsibilities: Participate in intake assessment completed by the Mental Health Consultant with caregivers and child. Observe the parent-child interaction during intake and in the classroom. Complete E-enrollment paperwork. Facilitate entry of child into the program collaboration with transportation and classroom staff. Provide ongoing case management and manage a caseload of ECLIPSE children and families. Provide home visits with parents of ECLIPSE children at a minimum monthly to address positive parenting skills and implement behavior plans consistent with the classroom. Implement identified Evidences Based Practices consistent with the treatment plan. Monitor family's need for ECLIPSE transportation and communicate with transportation liaison and document in child record. Provide classroom support and follow up with classroom staff regarding classroom interventions, children's progress or concerns, and family needs. Make referrals to community services whenever necessary. Meet productivity standards of 50% of time in direct face-to-face services including home visits, MDT meetings, case management, classroom observations, etc. Maintain a caseload of 15-20 families. Facilitate multidisciplinary meetings with parents, sources of referrals and allied service providers to establish a treatment plan for the child. Facilitate MDT meetings every 90-days to review the treatment plan and progress or concerns in services. Ensure informal supports, schools, and allied services providers are included in the MDT. Complete all documentation according to agency policy and required by the contract. Document all home visits, community meetings, MDT meetings and other contracts on calendar daily Complete documentation within 48 hours of scheduled visits Keep ELMS/Documentation tracker updated as required Attend and plan for transition meetings in conjunction with Mental Health Consultant upon families exit from ECLIPSE Document all phone calls, case notes, contacts with allied service providers and other treatment information in the Electronic record according to the ECLIPSE policies and procedures. Maintain communication with assigned social workers and referral sources regarding progress and concerns.
