Job Overview

Provide Social Work services to patients and families through the Cancer Center Program.

Qualifications

  • Master’s degree in social work (MSW) from a school fully accredited by the Council on Social Work Education

  • Current valid unrestricted certification as a Licensed Clinical Social Worker with no adverse actions.

  • Experience: Experience in clinical social work, either through a master’s-level practicum or 2 years post-MSW experience, within the last 3 years, in a clinical/treatment setting.

  • Basic Life Support certification

  • Competent in using electronic medical record systems; knowledge of computer operations and competency in the use of basic word processing, data entry, and automated medical records.

  • Health Restrictions: Employees will have no health or physical disability restrictions that interfere with the performance of duties.  This position has a mandatory seasonal influenza vaccination requirement and is therefore subject to annual seasonal influenza vaccinations.  The additional immunization/ screening requirements for the risk of exposure to hepatitis virus (Hep B) and screening for tuberculosis (TB).

Responsibilities Include but are not limited to:

  • Coordinate appropriate referrals to community and other Government agencies for services.

  • Serve as patient navigator for medical oncology physicians, proactively identify and evaluate patients and families for case management from a variety of sources such as through discharge/disposition planning, referrals, the Medical Evaluation Board (MEB) process, the healthcare information system, employers and facility staff;

  • Conduct systematic, ongoing, thorough collection of patients’ physical, emotional, psychological, social, and medical status and information to include direct patient contact and other relevant sources such as professional and non-professional caregivers, medical records, family/caregiver interviews;

  • Coordinate care with multiple providers across all levels and sites of oncologic care using a variety of media (digital, fax, telephone, paper) and must facilitate/coordinate outside referrals, assist patients in making outside appointments, independently follow up on completion status of outside studies and reports that were initiated by providers and shall evaluate care and outcomes to ensure timely and appropriate provision of services.

  • Facilitate and complete coordination of referrals and medical report/record/slide transfers to/from outside providers. HCW shall complete medical record documentation and coding, and designated tracking logs and data reporting as required by local MTF/AF/DoD instructions, policies, and guidance by the Cancer Center Director.

  • Attend weekly Tumor Board meetings, and quarterly Cancer Committee meetings, and coordinate leave schedule with Cancer Center Director to ensure continuity of patient care;

  • Manage certain aspects of the Cancer Care Program such as wellness/psychosocial initiatives (e.g. cancer support group meetings) and shall perform audits on psychosocial distress screening and survivorship plan completion rates and summarize findings in reports for presentation at quarterly Cancer Committee meetings.

  • Independently provide counseling and/or psychotherapy services to individuals, groups, couples, and families. Must practice within the bounds of the license or certification and the privileges granted by the MTF.

  • Provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.

  • Participate in professional peer review case conferences, research studies, or other organizations required at the MTF.
