CW SOLUTIONS LLC Mental Health Case Manager in WISCONSIN RAPIDS, Wisconsin

JOB REQUIREMENTS: We have a current opening for a Mental Health Case Manager position for the newly created Mental Health Treatment Court in Wood County, a collaboration between the Wood County Circuit Court, Wood County Human Services Department and CW Solutions. This position is located in Wisconsin Rapids. Under the direction of the Wood County Human Services Department Deputy Director, the Mental Health Case Manager provides and coordinates services to adult participants of the Wood County Mental health Court through case management and evaluation. The goal of the position is to help equip participants with the skills and resources needed to become and remain stable, law-abiding community members. The following duties are typical for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned. *Facilitate screening of prospective participants to evaluate for program eligibility and appropriateness. Promptly processes referrals by completing treatment screens efficiently and within time frame established by the agency. Make recommendations to the Mental Health Core Team. *Coordinate, schedule and conduct intake, risk/need assessment, evaluation and orientation with new participants. *Utilizing a comprehensive risk and need evaluation, work with multi-disciplinary teams to develop and implement a case management plan based on participant\'s needs. *Organize and coordinate Core Team staffing and Mental Health Court meetings. Facilitate meetings to discuss and problem solve how *Mental Health Court can support the participant in meeting their goals. *Document service notes timely and maintain participant files in accordance with agency standards. *Advocates on behalf of participants to assure participant needs including food, shelter, and insurance are met appropriately. Assist in coordinating treatment needs. *Meets with other community agencies/organizations, as directed, to assist in providing relevant programs/services for participants or to develop programs to meet other community needs. *Gathers and provides to administration such information as may be deemed useful for ongoing program evaluation and planning. *Perform regular, random and observed drug screens in accordance with agency procedures. *Assist with crisis situations and short-term interventions to ensure the participants health and safety needs are met. *Monitor program compliance to recommend evidence based therapeutic incentives and sanctions. Assures incentives and sanctions are given on a consistent and fair basis consistent with agency procedures. *Maintain and promote effective collaborative relationships with all community partners. Ability to work independently and as a team. *Complete all assigned trainings. *Ensures data is collected and documented within the time frame established by the agency. *Responsible to know and practice the safety policies of the County. Outstanding case management skills including application of holistic and comprehensive case management services utilizing Family Systems and Trauma Informed Care to. Case Management includes: intake, goal setting, long-term case plan development, case plan evaluation, progress monitoring, advocacy, and referral. 4. Effectiv ly respond to behavior crisis situations that builds safety for participants. 5. Ability to motivate and empower participants to achieve their personal goals. 6. Ability to work independently with strong sense of focus, task-oriented, nonjudgmental, participant focused and have clear sense of boundaries. 7. Excellent conflict resolution and/or mediation skills. 8. Ability to work in a variety of settings with culturally diverse families and communities with the ability to be culturally sensitive and appropriate, supporting participants within the context of their identity, family, community, tribe, history, culture, and traditions. 9.
