About the Case Management department:
We’re seeking a BSW Case Manager to join our team where you will help to support patients and families during a time of need. You will collaborate with the individual, family, physician, and interdisciplinary teams to coordinate healthcare services to promote optimal clinical, financial, and satisfaction outcomes. We are a tight knit and supportive group. This is an ever-changing job and have the ability to think outside the box.
Qualifications
- Bachelor's degree of Social Worker required.

Responsibilities
Provides counseling and crisis intervention to families and patients.
Conducts psychosocial assessments.
Serves as a resource to patients and families.
Facilitates/ Assists patients and families in open discussions regarding advanced care planning.
Being a patient advocate to ensure their needs are met and rights are protected.
