Responsibilities

Assure the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas, such as nuclear medicine, oncology, cardiology. Serve as a subject matter expert in pharmacy discipline and provide guidance across pharmacies and to clinical leadership.

  • Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems.

  • Recommend formulary alternatives when appropriate.

  • Compound and dispense medications for accurate delivery to patients.

  • Provide drug information and education to medical staff and patients.

  • Communicate with physicians and other clinicians as necessary to facilitate care.

  • Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies.

  • Assist with academic functions such as serving as a preceptor for pharmacy students.

  • May float to provide coverage of pharmacy needs.

  • Provide patient counseling when required.

Requirements

Licensure / Certification / Registration:

  • Pharmacist credentialed from the Kansas Board of Pharmacy obtained within 3 Months (90 days) of hire date or job transfer date.

Education:

  • Bachelor's or advanced degree in Pharmacy required.

Ascension Via Christi is offering an exciting opportunity for a motivated Ambulatory Care Clinical Pharmacist. The individual will focus on perpetuation and growth of our established Pharmacotherapy Clinic, providing pharmacy services in the clinic. The ongoing goal of the Pharmacotherapy Clinic is to support patient clinical management, education, and medication access in collaboration with multiple specialty medical clinics. The disease states which are currently a focus include adult and pediatric GI, hepatitis C, and pulmonology with possible expansion to other disease states in the future based on patient and organizational needs. With a focus on improving patient outcomes via the provision of collaborative drug therapy management (CDTM) services, the pharmacist will serve as a key member of the healthcare team and work alongside physicians under a collaborative practice agreement (CPA). The pharmacist will help each patient understand their condition and treatment components and ensure they have access to needed medications, working closely with our outpatient pharmacies. The successful candidate will have excellent communication and presentation skills, as well as knowledge and/or experience in developing and justifying ambulatory care pharmacy services. Involvement in education of physicians, medical residents, pharmacists, pharmacy residents, pharmacy students, nurses, and other healthcare professionals is a key responsibility. Opportunities for adjunct faculty appointment, precepting PGY1 and PGY2 residents, and the ability to conduct clinical-based research are available and strongly encouraged.

Requirements

  • Doctor of Pharmacy (PharmD) degree from an ACPE-accredited school of pharmacy

  • Valid license to practice pharmacy in the State of Kansas (or eligibility to obtain Kansas licensure)

  • Previous clinical experience in an ambulatory care setting and/or recent experience in direct patient care activities and pharmacotherapy

Preferred Qualifications

  • Completion of an ASHP-accredited PGY1 residency program with ambulatory care content or at least 3 years of practice experience in an ambulatory care (clinic) setting is strongly preferred

  • Completion of ASHP-accredited PGY2 residency training is also desired

  • Current Board Certification in Ambulatory Care or Pharmacotherapy or willingness to obtain certification in the next 12 months after attaining eligibility is desired

  • Previous experience working within a 340b program desired
