ESSENTIAL FUNCTIONS:
Coordinate and assist with community outings scheduled for military unit.
Assist social services staff with the discharge planning process.
Conduct/Lead groups on unit as assigned by director or therapists.
Provide orientation for new patients arriving on the unit. May conduct contraband checks when patient arrives to unit, as needed.
Audit and review charts as requested. May file loose paperwork and ensure compliance with all patient charts.
Assist Director and clinical staff with tracking and completion of assessments.
Assist with case management, as needed.
Provide transportation for patients, as needed.
Work with nursing and clinical staff daily for the unit needs.
Collaborate with treatment team members to assist with patient treatment and/or care goals.
Monitor the daily well-being of patients and identify need for social intervention.
Ensure all documentation regarding the program, such as care, treatment or incidents, is completed in a clear, concise, professional, and timely manner.
Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.
Strong effective communication
Military experience preferred.
OTHER FUNCTIONS:

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
High School diploma or equivalent required. Bachelor's degree in Human Services, Social Work or relatedfield preferred. One or more years previous experience in behavioral health with population of the facility preferred.
LICENSES/DESIGNATIONS/CERTIFICATIONS: Certification in specialty of program preferred. CPR and de-escalation/restraint certification required (training available upon hire and offered byfacility). First aid may be required based on state or facility
