Home Care Field RN

Westchester/Bronx

Schedule: M-F, 8-hour shifts

FlexStaff is seeking an experienced Registered Nurse for a Field RN position for our client, a leader in home health care. This is position will be covering cases in Westchester and The Bronx.

This position will manage all aspects of patient care related to services provided in the home.

Responsibilities:

  • Conduct home care assessments

  • Perform home visits

  • Complete all required documentation

  • Communicate effectively with Primary Care Physicians (PCPs)

  • Update medication profiles

  • Coordinate care with other providers

  • Review and complete case communication notes

  • Participate in discharge planning activities

  • Other duties as assigned

Requirements:

  • Active New York State (NYS) RN License

  • Home Health Care experience preferred

  • Experience working in a hospital setting

  • Valid driver's license and reliable transportation

When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).

The salary range for this position is $95,000-$110,000/year
