Maternal & Family Health Services Nutrition Counselor (CPPA) - Wellsboro in Wellsboro, Pennsylvania

Working to meet the health and nutritional needs of women, children and families with information, education, and quality care, MFHS is currently seeking a full time Nutrition Counselor to work in our Wellsboro WIC location.

 

*Job Summary: *

Responsible for the collection of data and the performance of screenings for client eligibility determination. Perform duties to assure the delivery of WIC benefits, nutrition education and non- high-risk certifications to eligible clients in accordance with organizational goals and program procedures.

 

Duties & Responsibilities:

  • Deliver and document nutrition education including providing breastfeeding promotion and support, preparing nutrition care plans, providing and documenting nutrition and breastfeeding contacts for nonhigh-risk participants, and prescribing and tailoring standard (nonhigh-risk) food packages for participant's medical/nutritional needs and preferences. Ensure all pieces of the nutrition interview are completed.
  • Provide and document appropriate and required referrals to participants including appropriate follow-up referrals.
  • Consult or refer to NEC, CPA, Breastfeeding Coordinator, or Designated Breastfeeding Expert in the agency when the situation is outside the CPPA scope or the CPPA is unsure how to handle a situation.
  • Verify, or confirm verification of, identification and residency of all applicants/participants; verify income eligibility; ask about voter registration status.
  • Conduct and document a thorough participant assessment and document identification of WIC risk code(s).
  • Explain the purpose of the program and provide an overview of what to expect during the appointment ("WIC Rules" and "Participant's Rights and Responsibilities"); provide food instruments and explain the food list and procedure for use of E-WIC card; , and schedule WIC appointments as per guidelines. 
  • Perform, record, and assess hematological and anthropometric measurements.
  • Promote and contribute to organizational and programmatic goals of MFHS.
  • Monitor and promote nutrition education, breastfeeding and outreach activities for participants in accordance with the current Nutrition Education, Breastfeeding and Outreach annual plans.

Qualifications/Requirements/Experience:

  • High School Diploma or G.E.D. required. Bachelor's or associate's degree in any field or have passed college course(s) in nutrition or dietetics from an accredited college or university is recommended.
  • One year experience as a Program Assistant or Breastfeeding Peer Counselor; or one year of experience in a related field.
  • Good verbal communication -- ability to articulate clearly and speak complete sentences using appropriate vocabulary and grammar to obtain information, explain procedures or processes, and/or persuade others to accept or adopt an opinion or action.
  • Bilingual (English/Spanish) preferred.
  • Ability to perform operations with units involved in measuring and weighing.
  • Basic math skills such as adding, subtraction, division and reducing fractions.
  • Exceptional customer service skills and client-centered service delivery.
  • General understanding of medical office processes and social ervice delivery.
