Prime Care Coordination Nurse Educator in Webster, New York
Summary:
Under supervision of the Manager of Nursing Services, collaborates with the training department to facilitate medication certification training and other health-related training to employees. Assist with placing PPDs for onboarding staff. Serves as a leader and assists the nurse managers to ensure the safe, effective, and productive treatment of individuals at all times. This position has the opportunity to create performance enhancing learning opportunities for staff and to ensure quality care for the individuals CDS Monarch supports. Supporting nursing management
Essential Job Functions:
Coordinated with CDSLT Training Department to provide or coordinate required education and training to staff including medication certification, new employee orientation, PPDs and other specific health-related needs.
Assist with on-boarding and mentoring of new nursing staff.
Consults with Manager(s) of Nursing to provide additional training and hands on skills practicums to nursing and direct care staff.
Remains aware of changes and requirements for OPWDD and other regulatory bodies and assists in maintenance and revision of clinical policies and procedures as needed.
Provides coverage for nursing staff who have planned or unplanned time off.
Become the subject matter expert with electronic medical record systems and assist with implementing systems agency wide.
Assist in maintaining all required documents per Agency policy and procedures.
Completes targeted reviews and internal audits for the purpose of identifying and managing quality issues as assigned.
Serves on assigned committees.
Participates in the identification of complex medical cases to ensure coordination of quality care.
Adheres to all agency and program policies and procedures.
Performs all other necessary duties relevant to the position as requested by the supervisor.
Advises direct supervisor of any obstacles to achieving responsibilities.
Knowledge, Skills, and Abilities
Ability to work independently and motivate others.
Ability to communicate effectively, both orally and in writing.
Proficient in time-management skills.
Proficient computer skills, including experience with Microsoft Office and EHR software.
Education and Experience:
Must be licensed and currently registered as a Registered Nurse by the State of New York State Education Department
Specialized training or one year of professional experience in treating those with developmental disabilities preferred.
Prior training/teaching experience in related field preferred.
Must be at least 18 years of age.
Reliable transportation (NYS Drivers’ Licensed required).
All experience and education requirements, except when required by federal, state, or local laws or requirements, may be waived at the discretion of management with the approval of the Executive Vice President, in collaboration with Human Resources.

Ability to sit continuously
Ability to reach above shoulder level
Ability to turn/twist upper body
Ability to use hand for repetitive action and fine manipulating for the purpose of keyboarding
Corporate Qualifications/Expectations:
Must meet applicable Vehicle Operator Requirements. (NYS Driver’s License required)
