UnitedHealth Group Manager, Field Social Workers - Washington DC in Washington, District Of Columbia

As a team member of our Optum At Home product, together with an interdisciplinary care team we help patients navigate the health care system, and connect them to key support services. This preventive care can help patients stay well at home.

The schedule for this position is Monday - Frida normal daytime business hours. The majority of your time will be working from home. You will be required to be in the field with your staff in the Washington DC area as needed for training, etc.

If you are located within Commutable distance to Washington DC, you will have the flexibility to work remotely* as you take on some tough challenges.

Primary Responsibilities:

  • Clinical operations across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating)

  • Case management and coordination of care

  • Takes lead role in setting direction and participating in or developing new programs

  • Sets team direction, resolves problems and provides guidance to members of the team

  • Ensures team meets established performance metrics and performance guarantees

  • Adapts departmental plans and priorities to address business and operational challenges

Required Qualifications:

  • Masters in Social Work Degree or other related clinical field

  • Active and unrestricted LICSW or LGSW license in Washington DC

  • 3+ years experience as a manager

  • 3+ years experience in a hospital setting, acute care, direct care experience or experience as a telephonic Case Manager for an insurance company

  • Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment

  • Ability to be in the field in Washington DC as needed

Preferred Qualifications:

  • Certified Case Manager (CCM)

  • Home Health experience

  • Washington DC Medicaid knowledge

  • Background in managed care

  • Case Management experience

  • Experience/exposure with discharge planning

  • Experience in utilization review, concurrent review and/or risk management

  • Resident of DC

  • Solid organizational skills and multitasking abilities will be keys to success

Washington Residents Only: The salary range for this role is $88,000 to $173,200 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
