UnitedHealth Group Manager, Field Case Management - Washington, DC in Washington, District Of Columbia

As a team member of our Optum At Home product, together with an interdisciplinary care team we help patients navigate the health care system, and connect them to key support services. This preventive care can help patients stay well at home.

The schedule for this position is Monday - Frida normal daytime business hours. The majority of your time will be working from home. You will be required to be in the field with your staff in the Washington DC area as needed for training, etc.

We are looking for a creative, experienced Manager to be a mentor and knowledgeable resource to their team. Someone who is innovative and a self-starter is a must! This position will oversee Care Coordinators working with members who receive Home Care based services. The Manager will ensure compliance with the contract requirements for the assessment and care of the population, as well as provide strong, encouraging leadership to the team.

Primary Responsibilities:

  • Demonstrates UHG cultural values, including modeling ethical behavior, delivering value to customers, leading and developing people, leading change and innovation, driving sound disciplined decisions and driving high quality execution

  • Ensures compliance of Case management and coordination of care per policies, procedures and contractual requirements

  • Ensures consistent leadership and direction for team

  • Sets team direction, resolves problems and provides guidance to members of the team

  • Takes lead role in setting direction and participating in or developing new programs

  • Clinical operations across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating)

  • Interprets complex information, simplify, clarify and communicate effectively to others

  • Ensures team meets established performance metrics and performance guarantees

  • Adapts departmental plans and priorities to address business and operational challenges

Required Qualifications:

  • BSN

  • Current, unrestricted RN license in Washington DC

  • 3+ years of experience in a hospital setting, acute care, direct care experience or preferably in field-based care coordination

  • 2+ years of Supervisory/Management experience

  • Proficient with Microsoft Office, with the ability to navigate a Windows environment

  • Ability to be in the field in Washington DC as needed

Preferred Qualifications:

  • Case Management experience

  • Experience in utilization review, concurrent review and/or risk management

  • Home Health experience

  • DC Medicaid knowledge

  • Background in managed care

  • Excellent verbal and written communication skills

  • Solid organizational skills and multitasking abilities

  • Reside in Washington DC
