BAYFIELD COUNTY Case Manager-Coordinated Services Team in WASHBURN, Wisconsin
JOB REQUIREMENTS: To provide social work services, including acting as the primary case manager for the Coordinated Services Team (CST) program; and on a more limited basis, conducts investigations, provides on-going case management, ACCESS and Intake for Child Protective Services, Child Welfare and Juvenile Justice to children and families in Bayfield County. Examples of Duties: 1. Coordinates teams and services for children with complex behavior health needs. 2. Primary case manager for the Coordinated Services Teams (CST) Program. 3. Develops teams of family members, service providers, and others who will work to design and carry out a coordinated service plan for children in CST that will meet their need with community-based support. 4. Provide Child Protective Services investigations and ongoing case management services. 5. Provides ACCESS services related to child abuse/neglect, child welfare, and law enforcement referrals. 6. Provides assessment, conducts home visits and supervised visitation, provides case management services to children, individuals, and families for court-ordered and voluntary cases related to child protection, child welfare, delinquency, and status offenses. 7. Provide children\'s Juvenile Court Intake services for the purpose of screening children/youth taken into custody. 8. Conduct intake inquiry interviews with children, youth, and families 9. Make decisions/recommendations regarding law enforcement and non-law enforcement intake referrals including case closure, informal disposition, deferred prosecution, or court petitioning. 10. Determine the range of services needed by individuals and families through interviews, home visits, collateral contacts, etc. 11. Provides families with information specific to their financial obligations for repayment for services provided. 12. Attends court hearings and provides written reports and testimony. 13. Monitors compliance with court orders 14. Coordinates services with other agencies/disciplines/community members such as law enforcement, schools, attorneys, mental health, and substance abuse service providers, etc. 15. Provides evening and weekend on-call services including assessment, provides crisis intervention, takes and transports children subject to temporary physical custody orders, makes custody decisions and referrals. 16. Completes case-related documents including correspondence, case decisions, and general documentation within specific timelines. 17. Arranges evaluation services to assist with family assessment decisions. 18. Prepares families for court hearings by explaining the process and by sharing recommendations. 19. Participates in section and department meetings, work-related committees, and trainings. 20. Provides informational presentations to other staff, professionals, and community groups related to department programs and policies. 21. Participates in supervision/consultation. 22. Performs other duties as assigned. ***** OTHER EXPERIENCE AND QUALIFICATIONS: 1. A minimum of a four-year degree from an accredited college social work degree preferred but will consider sociology, psychology, criminal justice or another approved human services program. 2. Completed most requirements and actively working toward becoming a certified social worker in the state of Wisconsin but not currently eligible for a training certificate or temporary license. 3. Minimum of one-year experience providing social case management to children and families 4. Experience in computer use and keyboarding (40 wpm) including Microsoft software including word processing, spreadsheets, e-mail, etc. 5.
