Job Summary and Qualifications
The Licensed Mental Health Provider provides clinical services in accordance with their scope of practice to patients and their families including but not limited to assessments, psycho-education, treatment planning, discharge planning and therapeutic counseling support through evidenced based interventions in individual, family and group therapy.
What you will do in this role:
Conduct individual, group and family psychoeducational and process groups in accordance with state regulations
Complete assessments, group notes and all other documentation timely and in compliance with requirements (as appropriate to scope)
Identify risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement)
Develop treatment plans and attend treatment planning meetings and reviews on an ongoing basis.
Understand state commitment processes, abuse reporting requirements, duty to warn and guardianship processes. Assists with documentation as required.
Assess, collaborate, and coordinate patient’s discharge planning needs and readiness for discharge with the multidisciplinary team.
Develop and maintain a community resource guide including access to public assistance/alternatives and facilitate and participate in Community Education, Activities and Support Groups (as appropriate to scope)
Serve as an educational resource to patients, families, interdisciplinary team members, payer representatives, and other parties as appropriate regarding behavioral services
Review, coach and assist with training of new staff/interns as assigned within approved practice and program guidelines.
Required Education & Experience:
• Masters in Social Work (or related field) Required
• Experience in mental health or related field Preferred
• License Master’s Social Work (LMSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT) and Licensed Professional Counselor-Associate (LPC-A) Preferred
