Queen's Health System Clinical Pharmacist - Wahiawa Pharmacy (Full-Time, 40, Varied Shifts) in Wahiawa, Hawaii
RESPONSIBILITIES
I. JOB SUMMARY/RESPONSIBILITIES:
• Implements, maintains and provides clinical pharmacist services including: clinical, educational, informational and distributional functions for newborn, infant, pediatric, adolescent, adult and geriatric patients of The Queen’s Medical Center (QMC).
II. TYPICAL PHYSICAL DEMANDS:
• Standing, sitting, stooping, bending, walking, finger dexterity, seeing, hearing, speaking.
• Frequent: pushing/pulling usual weight of up to 40 pounds of force, lifting or carrying up to 30 pounds, reaching above, at and below shoulder level, frequent gripping of an object.
• Continuous: repetitive arm/hand motions.
• Occasional: kneeling, crawling, climbing stairs, walking on uneven ground, squatting, static gripping of an object for prolonged periods, pushing/pulling up to maximum 50 pounds.
• Operate pagers, computers, printers, typewriters, telephones, fax machines, dispensers, laminar flow hoods, TPN compounding machines and other pharmaceutical and office equipment.
III. TYPICAL WORKING CONDITIONS:
• Not substantially subject to adverse environmental conditions.
• Subjected to noise/vibration from air conditioner, printers, time stamp machines, pneumatic tubes, telephone, etc.
• Exposure to body fluids, chemicals, some contact with patients with communicable diseases.
• May be required to work with hazardous drugs, depending upon area of assignment.
• Work environment is fast paced, congested and may be stressful due to workload.
• Interacts with patients, nurses, physicians and the general public.
• Works on multiple tasks at the same time.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
• Current Hawaii State license as a Pharmacist.
• Doctor of Pharmacy degree preferred.
• Post-graduate clinical residency accredited by the American Society of Health-System Pharmacists preferred.
B. EXPERIENCE:
• Clinical hospital pharmacist experience preferred. Preferably with experience in rounding with a medical team, monitoring of patient drug therapy, pharmacokinetic dosing and patient counseling and education.
• Experience in unit dose and IV services preferred.
• Ability to communicate professional expertise to all levels of the health care team.
