Ridgeview Medical Center Palliative Care Physician in Waconia, Minnesota
A member of the palliative medical team, responsible to the consultant, - providing the medical component of the multidisciplinary team. Ensuring that all dimensions of patient and family care (physical, emotional, social, practical, and spiritual and information) are considered, to assess and address these needs in accordance with the wishes of patient and family in collaboration with other members of the multidisciplinary team. Perform histories, examinations, assessments and plans for Palliative Care outpatients and inpatients and coordinate all evaluations, diagnostic testing, clinical follow up, and care coordination. Help facilitate Palliative Care admissions, post discharge care and specialty consultations as needed. Participate in evaluation and completion of consults for Palliative Care Participate in Interdisciplinary Team meetings Comply with all pertinent medical staff guidelines, policies, and procedures Maintain current requirements for credentialing and privileging including necessary license and certifications.
