Currently we are seeking a Social Worker for our program within our Primary Care outpatient practice, in Downtown Buffalo, NY!
The primary purpose of this position is to provide patients/caregivers with supportive counseling, psychosocial assessments, therapeutic interventions, education, and community resource support information.
This is a Monday through Friday position, scheduled from 8am- 4pm.

Essential Duties:
Provides psychosocial support and crisis management for patients throughout the continuum of care, including but not limited to diagnosis, living with chronic disease, recurrence, end-of-life, and survivorship.
Participates in multidisciplinary program development and evaluation, treatment modalities and emergency crisis/response.
Acts as a liaison for appropriate referrals to additional services including but not limited to psychiatry, psychology, supportive and palliative care, pain management, spiritual care, patient navigation and patient, family, and community education.
Collaborates with and promotes teamwork among all member of the healthcare team and openly expresses ideas for new programs or enhancements to current programs and services.
Utilize social work theory, principles, and methods to provide strength-based assessment, evaluation, formulation, and implementation of a treatment plan to address the psychosocial aspects of illness and its impact of the patient and caregivers.
Participates in/ assists with the planning and implementation of performance improvement activities to promote continuous improvement in care delivery.
Assists in education and training programs, policy writing, and research/program development.
Deliver exemplary customer service to provide a positive patient experience across the organization.
Perform other duties assigned by management
Pay Range: $55,000-$70,000
Minimum Qualifications:
Master’s Degree in Social Work (MSW) and must have NYS licensure as LCSW-R or LMHC
Two years Clinical Social Work required
