Position Overview: The Long-Term Service Support Coordinator with be working with a multitude of diverse partnering agencies across both health and social service disciplines. This is dedicated to promoting consumer choice and self-management as well as greater community inclusion, access, integration, mobility, and robust livability.
Position Responsibilities: Responsible for providing comprehensive and enhanced person-driven supports to enrollees, ages 3 to 64, that are participating in the Commonwealth's Long-Term Services and Supports Community Partnership program. You will also facilitate enrollee use of assistive technology, sustain enrollee engagement and encourage problem solving, manage date and supporting outcomes evaluation. Please note: home visits are required.
Qualifications:
•Bachelor's degree in Human Services or a related field or
•An Associate's degree and at least one year professional experience in the field; Community Health Worker (CHW) or similar certification
• Bilingual candidates will be given priority consideration
•Previous work experience working with persons with disabilities a plus (children and adults)
•Detail oriented
•The ability to appropriately handle confidential and personal information
•Strong writing and communication skills
•Proficient computer skills
•Reliable transportation and a valid driver license to make visits throughout the GLSS service area
Hours 40 hours per week, Monday through Friday
