The Patient Care Coordinator serves as the extension of the company's brand by providing exceptional customer service over the phone and on video. They will help create an excellent patient experience by presenting a positive and professional disposition throughout the entirety of the patient engagement process. They will be responsible for performing virtual front desk activities including the telephones, scheduling appointments, chart preparation, and a variety of administrative and clerical support services. This high-performing team works together as one unit, utilizing teamwork, fostering relationships and taking pride in always bringing their personal best to work each day to support Level2’s clinical teams and members.

PCCs serve as front and back office staff for providers, check out their video visits, schedule visits with members of the care team, request and release medical records, complete prior authorizations, answer incoming calls to clinic and transcribe the health history form (once received from member) into the EMR. The Patient Care Coordinator assists all care team members with scheduling or rescheduling patient appointment requests.

Primary Responsibilities:

  • Coordinate and implement patient intake and patient care logistics.

  • Answering incoming phone calls and be able to direct callers to appropriate staff or able to complete caller requests.

  • Provide support to provider team and nursing staff

  • Place outbound calls to patients for scheduling appointments and consultations

  • Place outbound calls to pharmacies or local PCPs as directed by clinicians

  • Identify and assess patient needs seamlessly to achieve customer satisfaction

  • Provide accurate, valid, and complete information by using the right methods/tools

  • Handle patient complaints, provide appropriate solutions and alternatives within the time limits; follow-up with appropriate team members and patient to ensure resolution

  • Join provider video visits for check out to schedule follow-up appointments as directed

  • Complete EMR documentation/registration and utilize documentation systems for all patient encounters

  • Comply with and follow policies and procedures as outlined by Level2.

  • Maintain the perseverance to drive and sustain the changes that occur at Level2 while being resilient and flexible

  • Active participation in weekly team meetings, care team rounds and ONE team meeting

  • Other administrative tasks as needed

  • Able to meet the needs of the patient by processing requests and referrals, providing efficient follow-up with the patient, pharmacy or other entity, as needed.

  • Strong commitment to Customer Service and Excellence in healthcare.

  • Ability to communicate clearly and effectively to both patients and employees, both verbal and written

Required Qualifications:

  • High School Diploma/GED (or higher)

  • 1+ years of virtual/telehealth and customer service excellence experience

  • 1+ years of strong phone contact handling skills and active listening of both patients and employees

  • 1+ years of multi-task, prioritize, and manage time effectively

  • 1+ years of strong team player, organized, efficient and have flexibility to handle rapidly changing processes, processing requests, referrals, and follow ups

  • Intermediate proficiency in Microsoft Office Suite tools, EMR, and Teams communication software

Preferred Qualifications:

  • 2-year college degree

WORKING CONDITIONS:

  • This position is a telecommuting, work-at-home position, to comply with HIPAA regulations, applicants must have a dedicated office space and the ability to secure confidential information

  • The physical environment requires the employee to use a computer with high-speed internet

  • Minimal travel is required (possibly one time per year)

California, Colorado, Nevada, Connecticut, New York, New Jersey, Rhode Island, Hawaii, Maryland, Washington, or Washington D.C Residents Only : The hourly range for California, Colorado, Nevada, Connecticut, New York, New Jersey, Rhode Island, Hawaii, Maryland, Washington, or Washington D.C residents is $16.00 to $28.27 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
