About the Position

At Goodwin Hospice, we're seeking a Social Worker who will be responsible for providing hospice social work services as part of the interdisciplinary team’s plan of care to meet the goals and needs of patients and their families at end of life; assisting in effective transition management for hospitalized patients; and assisting in the implementation of advance care planning.

Duties

  • Assists in admissions to hospice team and communicates initial assessment to hospice team.

  • Offers direct social work interventions to patients and their families and provides appropriate measures for helping in their personal adjustment to end of life issues.

  • Counsels with hospice patients and families to enable them to discuss and understand feelings, attitudes, fears, and anxieties related to problem solving, and to explore alternative solutions.

  • Participates in the development and implementation of the plan of care as it relates to psychosocial aspects.

  • Attends bi-weekly interdisciplinary group meetings and other meetings at the direction of the clinical supervisor.

  • Provides anticipatory bereavement counseling as identified at admission to hospice and partners with the Bereavement counselor on a plan of care.

  • Ensures effective transition management between hospital and home by visiting patients, as needed, in the hospital and meeting with discharge staff.

  • Participates in the ongoing implementation of advance care planning.

  • Educates hospice volunteers regarding roles and responsibilities in working with hospice patients.

  • Assists the clinical supervisor, administrator and medical staff in planning and implementing outreach programs to the community.

Requirements

  • Must be proficient in computer skills and able to learn at least two electronic medical record software.

  • Preferably a Master of Social Work degree from an accredited university and, if not licensed, is willing to work toward licensure.

  • Three (3) years of relevant experience in Social Work preferred; prefer at least one (1) year in hospice.

  • Must possess ability to deal tactfully and effectively with patients, staff, family members and the public.
