Position SummaryThe Health Coach – Pre-Diabetes (Pre-D) Program is responsible for implementing a CDC-approved Diabetes Prevention Program. The goal of the program is to prevent or delay type 2 diabetes by supporting and guiding members through a curriculum that teaches effective lifestyle change activities. This position collaborates with multiple internal teams (data, marketing, other healthcare services’ teams), external entities (members, employer groups, consultants), and community-based organizations to ensure the member successfully enrolls, engages and completes the Pre-D Program. In addition to leading a Pre-D cohort, the Health Coach operationalizes the Pre-D program per Diabetes Prevention Recognition Program (DPRP) standards.T his is a FT WFH role. Pay Range$30.13 - $37.66 ​​​annually Actual pay is based on qualifications. Applicants who do not exceed the minimum qualifications will only be eligible for the low end of the pay range.

Required Skills, Experience & Education:

  • Bachelor’s degree in public health or equivalent experience and training.

  • Completion of online CDC-approved Pre-D Lifestyle Coach training required within 90 days of hire; ongoing refresher or advanced coach training required to remain compliant with DPRP standards. Employer will cover cost.

  • Ability to work collaboratively and independently.

  • Excellent communication, interpersonal, and presentation skills.

  • Strong cultural competence and understanding of the importance of using Diversity, Equity, and Inclusion (DEI) concepts to guide and inform work.

  • Thorough knowledge and understanding of health behavior change theory.

  • Strong familiarity with Microsoft Office products.

  • Ability to learn proprietary databases and maintain confidentiality.

  • Strong organizational and analytical skills.

Preferred Skills, Experience & Education:

  • Certified Diabetes Educator, board Certified Health Coach (NBC-HWC); bachelor’s degree, in healthcare-related field.

  • Completion of online CDC-approved Pre-D Lifestyle Coach training, refresher courses and/or advanced coach training.

  • Experience in health coaching or field related to health coaching.

  • Experience in health insurance.

  • Speaker of both English and Spanish Languages

Primary Functions:

  • Ensures the Pre-D program aligns with the National Diabetes Prevention Program Lifestyle Change Program and adheres to a CDC-approved curriculum that complies with the DPRP standards.

  • Conducts outreach, and group health coaching sessions with Pre-D program eligible members via phone, email, mail and virtually to support member recruitment, enrollment, retention and completion of the Pre-D program.

  • Utilizes motivational interviewing, peer-to-peer learning, interactive learning and other techniques/strategies to ensure the CDC-approved curriculum is followed, members stay engaged and committed to activities for effective lifestyle change. Makes internal and external referrals, as needed.

  • Routinely documents member progress, records and verifies the required member data to submit externally (CDC, employer groups) and report internally to various committees, Supervisor and other leaders, as needed.

  • In collaboration with the Supervisor and data team, ensures all data is submitted accurately and timely.

  • Develops and monitors various key performance indicators including outreach, enrollment, attendance, retention, completion, disenrollment and health outcomes.

  • Monitors and shares data specific to key performance indicators, program milestones, health outcomes, associated incentives, invoices and other data, as needed.

  • Maintains current understanding of CDC’s DPRP standards, how to access the CDC and shares pertinent information with others, as needed.

  • Complies with all applicable laws and regulations, including those governing participant privacy and data security.

  • Ability to work a flexible schedule including early morning, over the lunch hour and early evenings to meet the needs of program participants.
