The Data Coordinator is responsible for obtaining, verifying and managing call management and on-call data. This includes data collection, data verification; data assurance and data analysis. The Data Coordinator is responsible for assisting administration (defined as but not limited to quality and outcomes management, medical staff department/committees and other hospital departments) by generating reports for process improvement and quality outcomes data management. He/She is additionally responsible for gathering data necessary to meet requirements and requests for regulatory agencies as determined by the hospital's management.

The data coordinator fulfills a variety of roles to provide continuity of care for patients. The data coordinator is an essential component of the transplant team and works in conjunction with clinical, financial and scheduling colleagues to assure patient records are requested, obtained and placed into the EMR in a timely and efficient manner for review and documentation. Duties include: oversight of patient medical records, creating patient education packets, responding to urgent records requests, setting up the patient education class and validating patient consents. Additionally, the data and support coordinator will help facilitate the referral to evaluation process in partnership with the Transplant Coordinator, LVN, scheduling coordinator, referral coordinator, and financial coordinator. S/he provides stellar customer service when interfacing with patients in person and over the phone, alerting clinicians when patient testing is uploaded, escalating urgent issues, and engaging outside facilities for patient medical records. The data management coordinator is also expected to problem-solve in real time and provide appropriate direction and support to patients and staff in regards to medical records. The data management coordinator will participate in team meetings and discussions related to complex cases and also advance operational throughput and process improvement to benefit patient care and record retrieval. S/he should possess analytic skills and be able to evaluate/analyze information, patient charts and physician notes to effectively obtain and upload patient records and convey/communicate information as needed to others. S/he should have excellent verbal and written skills and an ability to easily learn hospital and transplant IT systems as they relate to record retrieval. The data coordinator acts as the liaison between clinical, scheduling and quality teams to assure patient records are being effectively obtained and placed in the EMR to the correct location which is compliant with transplant/hospital standards and legal requirements. Bilingual fluency is strongly preferred.

Essential Duties:

  • Follows all HIPAA related compliance by keeping patient clinical/personal information private. This includes, but is not limited to: Confirming a minimum of two patient identifiers prior to discussions with patients and staff, sending written or electronic communications and/or entering new information into electronic systems or paper charts. Prior to speaking with any patient representatives confirm the representative is noted in the EMR, or transplant EMR, to receive HIPAA related information. If a HIPAA violation occurs, communicating immediately with a supervisor, quality department representative and entering the violation into the risk management system

  • Acts as a business professional in interaction and appearance by providing superior customer service when interfacing with patients, peers, physicians, superiors and hospital personnel/leadership. Represents the transplant department in a positive and proactive way to others when discussing role and duties.

  • Demonstrates advanced knowledge of medical records processes and the role of the data management coordinator to secure and place medical records into the transplant and hospital EMR systems. Duties include, but not limited to: verifying uploaded records reflect initial request and correspond to the correct patient by using more than one identifier; uploading records via paper, fax or electronic transfer into the correct EMR folders with less than a 2% error rate; creating education packets for patients; maintaining/creating any paper charts as requested; gathering lab results and distributing per established protocols; escalating acute medical information and/or issues obtaining/uploading records to appropriate staff when necessary; following up with assignments per established individual/team metrics so patient care is not delayed/stalled; maintaining an ability to problem-solve and self-navigate workload, and throughput, to maximum efficiency and productivity without errors/mistakes

  • Secures and uploads patient consent forms. Affirms all consents are accurately completed, labeled, signed, dated and time-stamped. Works with supervisor and quality department to assure accuracy of data uploaded into the EMR. If errors are identified, collaborates with data management team and quality to establish new workflow and processes

  • Establishes a sense of comradery among peers and with supervisor to facilitate teamwork with others and throughout the department. Works well with others to problem-solve, provides coverage when necessary and assists clinical and non-clinical staff during acute and unforeseen circumstances to advance patient care Maintains intermediate knowledge of OPTN/UNOS policies and timetables as they relate to data documentation and medical records. Understands the role of CMS in transplant as it relates to duties and responsibilities. Maintains advanced knowledge of compliance, regulations and laws related to health information management.

Required Qualifications:

  • Req High school or equivalent

  • Req Associate's degree Or AHIMA certification required.

  • Req 2 years Experience in the medical field, including medical records and transplant

  • Req Excellent skills in the use of networked personal computers and data entry and data base relationships.

  • Req Detail oriented and aware of the importance and impact of data quality.

  • Req Must be able to use Microsoft Office applications and serve as a resource on the use of the application to staff.

  • Req Ability to become proficient in EMR and various IT systems required.

  • Req Demonstrates excellent verbal and written communication skills.

  • Req Able to function independently and as a member of a team.

  • Req Demonstrated ability to problem-solve and provide effective resolution

  • Req Excellent working knowledge of acute care demographic and clinical data elements, medical terminology and formulae.

  • Req Knowledgeable of JCAHO standards especially those related to Improving Performance.

Preferred Qualifications:

  • Pref Bachelor's degree In a health-related field

  • Pref Content knowledge of medical terminology and transplant process.

  • Pref Bilingual strongly preferred; if bilingual certified department translator strongly preferred.

Required Licenses/Certifications:

  • Req Fire Life Safety Training (LA City) If no card upon hire, one must be obtained within 30 days of hire and maintained by renewal before expiration date. (Required within LA City only)

The hourly rate range for this position is $19.00 - $29.77.
