About the position
The Clinical Liaison provides in-person discharge planning support to referred persons in local hospitals. The Clinical Liaison provides education, assessment, referral and admission to Goodwin Living service lines, as appropriate, and ensures effective transition management from hospitals to discharge destinations. This position is responsible for the utilization of health services to include but will not be limited to: Home Health, Hospice, rehabilitation and personal care services. This position requires travel throughout the Northern Virginia area.
Job Duties
· Manages a caseload of hospitalized patients; is responsible for a well-organized approach to discharge planning, medical goals for care and referral/resource coordination.
· Collaborates effectively with residents/patients/families and Goodwin Living staff members.
· Establishes and maintains effective working relationships with hospital providers across multiple hospital locations to increase hospital utilization of Goodwin Living services.
· Provides effective transition management between the hospital and discharge destinations; serves as a consistent and direct point of contact for resident, families and hospital providers during hospitalizations.
· Provides in-person and/or telephone follow-up for patients and residents according to program standards.
· Utilizes Goodwin Living and hospital electronic medical record systems, documents all necessary interactions according to applicable facility and regulatory requirements; tracks all necessary program data.
· Participates in interdisciplinary meetings to provide and receive necessary information on hospitalization updates and associated referrals.

Job Requirements
Registered nurse (RN) OR Master’s degree in Social Work from an accredited university; Licensed Clinical Social Worker (LCSW) in the Commonwealth of Virginia preferred.
Must be proficient in computer skills and able to learn at least two electronic medical record software.
Three (3) years of relevant experience preferred.
Must possess ability to deal tactfully and effectively with patients, staff, family members and the public.
