About the Position

Are you passionate about making a difference in the lives of older adults? Do you thrive in a dynamic environment where you can use your creativity and problem-solving skills? If so, Goodwin Living invites you to join our dedicated team as a Care Coordinator!

As a care Coordinator, you will be the primary point of contact for our members, ensuring their needs are met with exceptional service and care. You will develop and maintain a network of service providers, conduct assessments, and facilitate the delivery of services that help our members age comfortably and confidently in their own homes. Your role is crucial in achieving high member satisfaction within the Goodwin Living at Home program.

Key Duties:

  • Network Development: Collaborate with the Manager of Member Experience to build and maintain a network of top-tier service providers, including home care agencies and health care providers.

  • Member Assessments: Conduct pre-admission, annual, and as-needed assessments to determine the specific needs of each member, ensuring they receive appropriate services.

  • Member Engagement: Regularly communicate with members through visits, phone calls, and meetings to ensure their satisfaction and address any concerns.

  • Service Coordination: Manage the placement and quality monitoring of services, ensuring members understand their coverage and financial responsibilities.

  • Community Outreach: Participate in informational seminars and networking activities to enhance community awareness of our continuing care at home program.

Job Requirements:

  • Educational Background: Bachelor’s degree required; a degree in social work is preferred.

  • Experience: Experience with older adults and providing in-home services is a plus.

  • Skills: Strong communication skills, both verbal and written. Proficiency with Windows operating systems, MS Office, and email software.

  • Attributes: Ability to work independently, manage time effectively, and exhibit a high degree of initiative and creativity. Commitment to quality care and sensitivity to the needs of older adults.

  • Flexibility: Must be able to work flexible hours, including rotating after-hours on-call duties, and handle members’ needs during nights and weekends as required.

  • Travel Requirements: Must live in DMV area, work from home office and visit members in the Northern VA and Washington D.C. area as they age in place.
