Guthrie Nurse Case Manager - Hospice in Vestal, New York

The evening shift Hospice RN Case Manager for Vestal, NY is scheduled four 10-hour shifts working Wednesday - Saturday. Evening shift is from Noon -10:30 PM: Wednesday thru Friday and 10:00 AM - 830 PM on Saturday.

Who We Are:

The Guthrie Hospice Vestal Team is searching for a disciplined & compassionate, evening shift, Registered Nurse Case Manager. Our amazing team of Case Managers are critical thinkers that help provide a high standard of end of life care to patients in their homes. Position requires clinician with strong communication skills who is organized, flexible and who is seeking meaningful work.

Position Summary:

Under the direction of the interdisciplinary team, the Hospice Nurse works under the general direction of the Hospice Medical Director and the attending Licensed Independent Provider (LIP) in the provision of skilled nursing assessment, planning and care to maximize the patient’s comfort at end of life and support the patient’s families/ primary caregivers. The Hospice RN is responsible for identifying patient/family needs and for providing supportive care in accordance with the attending LIP’s orders, the interdisciplinary plan of care and Guthrie Hospice policies and procedures. Responsible for providing direct supervision of Home Health Aides and or LPN’s providing care for Guthrie Hospice.

Education, License & Cert:

Registered Professional Nurse currently licensed in New York. Knowledge of nursing theory and practice at a level normally associated with an associate degree. Baccalaureate Degree and/or Hospice and Palliative Care Nursing Certification preferred. Education and experience requirements may vary according to the combined background of the individual.

Requires valid driver’s license and reliable vehicle.

Experience:

  • One (1) year nursing experience in an acute care setting but a minimum of 3 years Med /Surgical Nursing or Similar Experience is desired.

  • Previous experience with end of life and /or chronic disease management is preferred.

  • Experience and skill in supervision desired.

Essential Functions:

  • Assessing the patient’s and the family/caregiver’s physical, psychosocial, bereavement, environmental, safety and developmental needs;

  • Developing an individualized plan of care, in conjunction with the hospice interdisciplinary team based on assessment, identification of needs, and patient and family/caregiver goals and preferences;

  • Providing care to patients and families/caregiver through the utilization of interventions and evaluation of outcomes of care;

  • Performing ongoing assessments and revision of the plan of care, with interdisciplinary collaboration, in response to the changing needs of the patient and family/caregiver;

  • Performing comprehensive assessment of pain/symptoms and developing individualized symptom management plan;

  • Anticipating, preventing, and treating undesirable symptoms;

  • Providing support, instruction, and education to the patient/family/caregivers;

  • Documenting nursing assessments, identified problems, measurable goals of care, limitations to provision of care, care interventions and response to care;

  • Coordinating all patient and family/ caregiver services with the hospice interdisciplinary team and other health care providers;

  • Consulting and collaborating with the hospice interdisciplinary team and others involved with the patient’s care;

  • Developing the hospice aide plan of care and providing direct and indirect supervision of aide at least every 14 days;

  • Maintaining the dignity of the patient;

  • Recognizing and supporting the patient’s and family/caregiver’s spiritual and cultural beliefs;

  • Providing holistic, family – centered care across treatment settings to ensure continuity of care;

  • Participating in the hospice program’s quality assessment performance improvement program;

  • Assessing the ability of patient and family/caregiver to safely administer medications and perform treatments;

  • Reporting abuse and neglect in accordance with state laws and regulations and Guthrie policy.

The pay range for this position is $31.42 - $50.11 per hour based on experience.
