Job Summary

The RN - Wound & Ostomy is responsible for assessing, planning, promoting and evaluating quality patient care. The Wound & Ostomy Nurse provides acute and rehabilitative care to people with gastrointestinal (GI), genitourinary (GU), and integumentary (skin) system wounds and disorders. They also provide direct care to people with abdominal stomas, wounds, fistulas, drains, pressure injuries, and/or continence disorders. Utilizing the nursing process in accordance with the standards established by the State Nurse Practice Act, Code of Ethics for nurses, ANA Standards of Care and the mission, philosophy, policies and procedures of MVHS.

The Wound & Ostomy Nurse assesses, plans, implements, and evaluates wound, ostomy and continence care throughout hospital areas to provide optimal patient care. May provide consulting to other MVHS affiliates. Works under the direction of the Education Services Manager or designee.

Core Job Responsibilities

  • Manages outcomes for wound, ostomy and continence patients across the continuum of care by providing, coordinating and supporting clinical aspects of direct patient care.

  • Acts as a specialty consultant to providers, nurses, and patients for provision of Wound & Ostomy care for all areas. Creates an atmosphere of learning to motivate staff to increase knowledge and skills.

  • Evaluates patients identified as “Very High Risk” (Braden Score 9 or less) per the Braden Report to determine if additional preventive care needed as well as validation of Braden care and interventions.

  • Identifies patients at risk for pressure injury development, implements preventive care per established policies in collaboration with unit RNs and monitors response.

  • Works effectively with other healthcare providers to assess, plan, implement, and evaluate patient care.

  • Collaborates with multidisciplinary teams to assess patient needs for nursing care, patient/family education and other services.

  • Evaluates the effectiveness of intervention/actions in relation to intended goals and modifies treatment approach as necessary. Revises patient care needs and plan based on evaluation of interventions in conjunction with patients, families and providers.

  • Coaches and assists team members to ensure optimum patient care and efficient use of resources.

  • Works closely with members of the patient care teams to ensure that personalized treatment plans are followed correctly to prevent future injury or infection.

  • Evaluates policies /guidelines for prevention and treatment and recommends revisions when warranted.

  • Provides adequate information and instruction to the patient and/or family. Identifies patient/family learning needs. Documents patient/family response to teaching when appropriate.

  • Collaborates with Discharge Planner to assure continuity of care through appropriate discharge planning, coordination of services and education.

  • Actively participates on the Pressure Injury Prevention Committee.

  • Demonstrates responsibility and accountability for own professional practice consistent with nursing and organizational standards.

  • Attends and completes required educational programs and keeps current in the practice of wound/ostomy care.

  • Participates in CQI and TQM concepts and practices modes of continuous quality improvement.

  • Complies with all regulations, nursing standards and organizational policies.

  • Assists in the orientation of new staff by sharing knowledge, experiences, and providing verbal encouragement.

  • Performs related duties as assigned.

Education/Experience Requirements

REQUIRED:

  • Two years registered nurse experience.

  • Bachelor’s Degree or expected completion within two years.

PREFERRED:

  • 5 years relevant clinical nursing experience

Licensure/Certification Requirements

REQUIRED:

  • Current New York State Registered Nurse license.

  • National Certification required as a Wound/Ostomy Nurse (e.g. WOCN, WCC or obtains within one year of hire).
