Through your work as a County Caseworker, you will have case management responsibilities for individuals and/or families receiving services from various systems and provider agencies. This is accomplished through coordination of service systems and providers, support provided to the individuals and families we serve by organizing and facilitating team planning meetings, linking individuals to community resources and services providers, monitoring the efficacy of services, and encouraging the individual's progress toward their desired outcomes.
Caseworkers serving Delaware County, seek to improve the lives of its more vulnerable residents; individuals who are living with intellectual and developmental disabilities, in order to provide the opportunity to lead a happy and healthy life of their choosing.
