GENERAL DESCRIPTION:\ \ The Family Support Specialist/Assessment Worker Flex Support Roleis cross trained in the FSS/FAW position and is responsible for initial identification, referral and coordination activities for high-risk families of newborns. This role is also responsible for any coverage needed within current Program Caseloads providing home-based prevention services focusing on supporting the parent child relationship, attachment, and family enhancement through child development education, play, goal setting, assessments, and coordination of services/activities for overburdened families. This position reports directly to the HF Supervisor.\ \ SUMMARY OF ESSENTIAL JOB FUNCTIONS:
- - Provides cross trained services in Family Assessment and Support wherever the program need is most identified.
{=html} <!-- --> - - Coordinates program activities with hospital, Child Protective Services, community based agencies and the public.
{=html} <!-- --> - - Make DCS referrals as appropriate.
{=html} <!-- --> - - Provide community presentations and community outreach activities.
{=html} <!-- --> - - Provide direct support to postpartum women identified as high risk.
{=html} <!-- --> - - Receive daily referrals from hospital personnel.
{=html} <!-- --> - - Administer the Healthy Families Screening Tool on all referrals to determine risk level and need for early intervention services.
{=html} <!-- --> - - Administer the Family Resilience and Opportunity for Growth (FROG) assessment on eligible referrals.
{=html} <!-- --> - - Maintain client records in accordance with program and funding source requirements.
{=html} <!-- --> - - Attend and participate in all scheduled Healthy Families seminars, conferences and in-service trainings.
{=html} <!-- --> - - Perform other related duties as assigned.
{=html} <!-- --> - - Conducts home visits virtually or in-person and assesses family needs and provide case management services when necessary, while promoting self-sufficiency.

{=html} <!-- --> - - Utilizes floor time to support play within the parent and child relationship.
{=html} <!-- --> - - Utilizes appropriate limit setting and boundaries.

{=html} <!-- --> - - Maintains necessary assessments and documentation in accordance with agency and funding source requirements.
{=html} <!-- --> - - Participates in and comes prepared for weekly supervision with Program Supervisor.

{=html} <!-- --> - - Attends and participates in all scheduled Healthy Families conferences, seminars, in-service training and parent events.
{=html} <!-- --> - - Maintains all Healthy Families quality assurance standards, i.e. Home visitation rate, etc.
