Unity House ESSHI Case Manager in Troy, New York

Responsibilities:
Provide supported service to individuals living with mental illness.
Provide support for the Empire State Supportive Housing Initiative.
Provide independent living education.
Advocate for consumers.
Assist with housing placement and referrals.
Crisis intervention to program consumers.
Coordinate care with other case managers and providers to wrap around services.
Promote a culture of inclusion and belonging.
Requirements:
High school diploma or GED and at least 2 years' experience working with adults living with a mental illness and/or other high-risk populations is required. Experience working with the homeless population a plus. College degree in related field preferred. Valid New York State Driver License is required. Reliable transportation. Ability to work independently and as a team. Bi-Lingual is a great asset!
