Per Diem - Behavioral Health Counselor – PROS Program

Hours Operation M-F 8-4:30

What you will do:

Samaritan Personalized Recovery Oriented Services (PROS) , is seeking an individual who is passionate about assisting individuals with mental health diagnosis in achieving their life roles. This individual will need to be skilled in case management, organized and self-directed and able to participate in working with a multidisciplinary team.

Responsibilities :

  • Plan and facilitate educational groups related to life skills and wellness self-management.

  • Case Management including engagement through face to face as well as phone outreach, community integration, linkage, and utilization of community resources.

  • Utilizing the EMR to document services provided including individual encounters, outreach, groups in a timely manner.

  • As a member of the multidisciplinary team, engaging in program planning, crisis intervention, engagement to support participant achievement in their life role goal.

  • Complies with all St. Peter’s Health Care Services.

What you will need :

  • minimum of HS Diploma or GED, 1 year experience working in an addiction or behavioral health related field.

  • Valid NY state drivers license and the ability to pass a employer drivers physical

"Pay Range: $17.28 - $26.67

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
