Are you passionate about making a positive impact on the lives of individuals in need? Join our team as a Case Manager at our Northwest Community Action Center in Toppenish, WA. We are seeking a dedicated individual to provide comprehensive case management services to our clients.

The Northwest Community Action Center works with schools and businesses to help families live better. We help youths and adults learn new skills, find jobs, and pay their power bills.

What You'll Do:

  • Build program-specific caseload by seeking referrals from providers and partners
  • Conduct intake screenings to assess clients' needs and determine appropriate case management services
  • Conduct in-depth client interviews to determine current status and barriers
  • Refer clients to appropriate agencies/services based on their needs
  • Assist clients with the self-referral process
  • Regularly meet with clients to monitor service plan implementation
  • Create, update, and maintain client service plans in accordance with guidelines
  • Establish and maintain relationships with service providers/agencies
  • Participate in meetings, including interdisciplinary team case reviews
  • Perform clinical responsibilities in alignment with regulatory requirements

Qualifications:

  • Bachelor's Degree in a social service field such as Social Work, Behavioral Sciences, Psychology, or Child Development OR
  • Associate's Degree with two years of relevant experience OR high school/GED with four years of relevant experience
  • Bilingual (English/Spanish) preferred at level 10 for bilingual differential pay
  • Valid Driver's License and proof of automobile liability insurance coverage required
  • Effective verbal, written, and listening communication skills
  • Knowledge of medical terminology and local social and health services
  • Ability to develop effective relationships with social service providers and partners
  • Basic proficiency with Microsoft Outlook, Word, and Excel
