Job Overview:
The Social Worker administers individual counseling to Caregivers who meet the specified criteria under the Title III E Caregiver Grant. Facilitate the monthly Caregiver Support Group.
Qualifications:
Required:
LCSW with at least 1 year experience in health care OR
LSW under the supervision of an LCSW
Scheduling Requirements:
Day Shift
Part-Time
10-15 hours per week
Essential Functions:
Assesses and evaluates each caregiver's personal history and that of the person being cared for
Individualizes a care plan based on evaluation of all aspects of a client's current needs for support and referral to appropriate agencies for assistance
Promotes program services to all health care providers in Ocean County
Properly records all elements of visit on client s chart
Researches new resources for caregivers in Ocean County
Facilitates weekly Caregiver's Support Group
Counseling to develop and enhance coping skills, communication techniques, and advocacy skills
Counseling for care planning and decision-making
Performs other duties as assigned
