ProMedica Health System Pathway Supervisor - Pathways - Full Time - Days in Toledo, Ohio
The supervisor for Pathways works directly with a team of Community Health Workers (CHWs) who deliver infant mortality prevention education and services to clients in their homes. Major duties include ongoing guidance and coaching to support adherence to quality standards, continuous quality improvement and individualized goal attainment, timely documentation review and billing procedures, as well as performance tracking and reporting. This position also serves as a liaison between Ebeid Children's Hospital and program funders at Hospital Council of Northwest Ohio.
ACCOUNTABILITIES
Conducts regular individual and group supervision sessions with CHW’s to meet individual and team caseload goals and quality metric standards set forth by funder.
Provides continual coaching to CHW’s to help improve time management skills and service delivery; conducts home visit shadowing to evaluate CHW’s performance.
Reviews and approves client documentation in a timely manner; monitors CHW caseloads to provide continuous quality improvement feedback to ensure timely billing approval.
Responsible for tracking expenses to maintain financial stewardship, tracking outcomes to ensure operational expenditures do not exceed income.
Leads team development and cohesion, monitors individual performance, provides guidance and support to grow CHW’s professionally; conducts annual performance reviews for all CHW’s.
Creates and supports connectivity between home visitation teams and departments within the hospital setting; represents ProMedica Ebeid Children's on both internal and community collaborative initiatives; grows community partnerships to improve services to pregnant mothers and families.
Generates reports on a bi-weekly, monthly, quarterly, and annual basis including payroll, fiscal and grant reports, as well as community health needs assessment data.
Other duties as assigned. Education: Bachelor's degree in social work, public health, or another human services-related field.
Skills: Must possess excellent time-management, problem-solving, and organizational skills as well as strong oral and written communication skills. Must have strong computer skills and be able to function independently with a commitment to high customer satisfaction.
Years of Experience: Three years as a CHW, home visitor in Pathways, Help Me Grow, or a similar in-home service delivery program.
License: Valid Ohio driver's license and insurance.
Certification: N/A
PREFERRED QUALIFICATIONS
Education: Bachelor's degree in social work, public health, or another human services-related field.
Skills: Experience working with Care Coordination System, Pathways HUB or OCHIDS is highly desirable. Leadership and coaching experience preferred.

Certification: Certified Community Health Worker or Ohio home visiting credential preferred.
WORKING CONDITIONS
Personal Protective Equipment: Gloves and/or face masks as needed.
Physical Demands: Walking, sitting, or standing for extended periods; must be able to move between multiple workstations/work sites and lift at least 25 lbs.
Travel: Occasional nights and weekends; travel to client homes and other off site local destinations approximately 15% of the time.
