    Resource Coordinator-IP

    1. Acts as liaison with the patient and family and the hospital.

    2. Collaborates with providers, the interdisciplinary care team throughout the health system, and community agencies as applicable.

    3. Reviews medical records and interviews with patients and/or family as appropriate; assesses and investigates patient's needs and acts accordingly.

    4. Helps coordinate plans of care, including physical, financial, and psychosocial needs.

    5. Supports the Care Manager with the needs of the patient for discharge planning needs.

    a. Demonstrates understanding of the physical and emotional need of a patient and/or family.

    1. Completes tasks as necessary to meet plan of care goals; tasks may include, but are not limited to:

    a. Assistance with arrangement of transportation

    b. Working with patients on financial assistance for medicine.

    c. Making referrals to community resources (DME providers, home health care, skilled nursing facilities, mental health providers, etc.)

    d. Collaborates with agencies such as, but are not limited to APS, CPS, etc,

    e. Assistance with advanced directives as applicable.

    1. Coordinates with RN-Care Manager to confer if interventions need to be modified or adjusted.

    2. Communicates and collaborates with the OP Care Coordination team when patients transition out of the hospital, if applicable.

    3. Participates in Care Coordination Rounds and Interpersonal Rounds daily, if applicable.

    4. Is knowledgeable regarding pertinent community agencies, institutions, and other resources.

    5. Maintains knowledge of value base programs to ensure appropriate use of payer and patient resources to promote high-quality, cost-effective care.

    6. Consistently demonstrates ability to balance workload and utilize time effectively as reflected in timely and smooth care transitions.

    7. Utilizes various methods of communication and assesses the effectiveness of each.

    8. Maintains accurate, concise, and timely documentation in the medical record and other department record keeping systems.

    9. Fosters preservation of patient's rights through concept of patient advocacy; possessing understanding and helping people of various backgrounds.

    10. Assumes responsibility for professional growth and development.

    11. Seeks assistance in meeting self-learning needs through in-service, continuing education, self-study, and demonstration.

    12. Shows an understanding of the principles of growth and development over the life span and possesses the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirement relative to his or her age-specific needs.

    13. Demonstrates ability to form positive, collaborative relationships with members of the health care team.

    14. Demonstrates initiative, adaptability, and dependability.

    15. Communicates effectively with others.

    Education, Training & Experience

  • Must have a degree in Healthcare related field

  • Must have active IN LPN certificate

  • Case Management Experience Preferred.

  • Hospital Experience

  • Must have active BLS certification
