As a Pharmacy Care Technician you will assist the pharmacist in collecting data and preparing reports for a medication reconciliation review. Working transition of care cases from hospital discharges and skilled nursing facility discharges. This includes accessing medical records from various electronic health records, internal and external. Making outbound calls to Optum members to gather further medication and medical history pertaining to the case. Attend and report on interdisciplinary team meetings to discuss Optum member inpatient stays at skilled nursing facilities. Collect and prepare data to complete a medication reconciliation while the member is in house. Build rapport with the physicians and internal workgroups to present any drug therapy issues that were identified, and collaborate to facilitate a safe discharge for our members. Data tracking and reporting, proper documentation, working within multiple platforms, and common Microsoft office products.

Primary Responsibilities:

  • Data entry for admission/discharge orders to create medication reconciliation report for pharmacist review

  • Contact members to collect details on their medications and medical history

  • Identify and address medication errors and discrepancies for the pharmacists to create accurate recommendations for member providers

  • Track and record data for skilled nursing admissions, ensuring all pharmacy components are completed during the course of the members stay.

  • Collaborating with internal and external work groups to facilitate safe discharge.

Required Qualifications:

  • High school diploma, GED or equivalent work experience

  • Pharmacy Technician licensed and certified in the state of Arizona

  • Proficiency with computer and Windows PC applications including ability to navigate and learn new and complex computer system applications

  • Data entry experience (preferably pharmacy/prescription)

  • Dedicated workspace – access to high-speed internet

  • Current resident of Arizona
