State of Massachusetts Lead Clinician - Clinical Social Worker (C) in Taunton, Massachusetts

The Department of Mental Health (DMH) On Grounds Program is seeking a dynamic human service professional, exceptional and experienced Clinical Social Worker, to function as a Lead Clinician. The Incumbent will evaluate individuals and will use their clinical skills and administrative expertise to direct the work of a multidisciplinary team of clinicians, nurses, mental health workers, peers, and student interns in meeting the mental health and daily living needs of each individual served by this program. Incumbent functions in both individual and group settings and performs other related duties required to ensure the effective maintenance and operation of the program and population served. Please Note: This position is 40 hours per week. The schedule is 8:30 am to 5:00 pm Monday through Friday. The days off will be Sunday and Saturday. _Duties and Responsibilities (these duties and responsibilities are not all inclusive):_ · Conducts ongoing assessment and provides integrated treatment planning, service delivery, admission and discharge planning in collaboration with all persons served, family members, and/or guardians in conjunction with DMH policy. · Serves as the primary liaison to the DMH Mental Illness/Problematic Sexual Behavior Team (MI/PSB) in the Southeast Area providing weekly and as needed updates regarding residents of the On Grounds Program identified with MI/PSB participating in MI/PSB treatment. · Participates in monthly specialized trainings to develop specialized skills for the treatment and risk management of individuals with MI/PSB. Assists in program development efforts regarding the initiation and implementation of MI/PSB service recommendations. · Functions as a Lead Clinician within the spectrum of state operated Adult Community Clinical Services which provides an integrated and flexible system of care that is driven by Clinical, Rehabilitative, and recovery expertise, conducts client screenings, as well as assessment of harm to self or others. · Serves as a member of the clinical team providing individualized services, supportive counseling, psychotherapy and wrap around services to individuals in in the least restrictive clinically appropriate setting when feasible; also providing psycho education, skill development, social opportunities and ADL training to persons served with chronic mental illness. Documents according to the CSP, as well as to share all updates and important health related information in the EMR. · Provides culturally and linguistically competent services that are also sensitive and responsive to religious practice, sexual orientation, gender differences, physical disabilities, as well as services that are age and developmentally appropriate, including transitional age youth and elders. · Supports and teaches the person served in the performance of daily living tasks by assisting in the acquisition of entitlements by navigating the legal and social service system in the community by making home visits and by networking with other social service agencies to maximize the individuals' potential for independent living. · Provide and/or facilitate internal trainings for staff as related to the clinical needs of the program residents. · Coordinates, facilitates, and supervises the clinical team and Social Work interns. · Practices the principles and concepts of psychosocial rehabilitation (such as empowerment, self-determination, dignity of risk, etc.) and their application in a community setting while promoting each individual’s growth and recovery, as well as cultivating and embracing peer-to-peer education and services. · Facilitates individual’s access to needed medical and dental services, as well as promotion of wellness activities and initiatives that enhance a person’s overall health and well-being. · Assists interested individuals in optimizing their work potential, by discussing with the Team and developing a plan for the appropriate referral to meet the persons served needs. · Ensures appropriate linkage in the community and actively collaborates in the effort; provides substance use/abuse intervention to the dually diagnosed. · Advocates for persons served by acting as a liaison between the individual and community to ensure that they are receiving appropriate service which may require prompt on-site community visits and referrals. · Responsible for completing fact-findings within program. · Assesses and responds to crisis and emergency situations by intervening immediately with appropriate clinical treatment, intervention, contacting the on-call supervisor or contacting the Emergency Services Program. · Documents all pertinent and necessary information in the medical record including mental status exams, screenings, comprehensive assessments, treatment plans, progress notes and social/psychiatric /medical histories. _Required Qualifications:_ * Those employees who elect to use a motor vehicle for travel must have a current and valid Massachusetts Motor Vehicle Operator's license or the equivalent from another state. _Preferred Qualifications:_ * Knowledge of community service network * Experience working with individuals with mental illness. * Given the population served, bilingual or multilingual fluency. _ About the On Grounds Program:_ On Grounds Program is part of the Taunton/Attleboro Site that transitions clients from an inpatient setting to a more independent community living setting. MINIMUM ENTRANCE REQUIREMENTS: REQUIRED WORK EXPERIENCE: At least two years of full-time, or equivalent part-time, professional experience as a clinical social worker after earning a Master's degree in social work. SUBSTITUTIONS: - A Doctorate in social work, psychology, sociology, counseling, counseling education, or human services may be substituted for the required experience on the basis of two years of education for one year of experience. - One year of education equals 30 semester hours. Education toward a degree will be prorated on the basis of the proportion of the requirements actually completed. REQUIRED EDUCATION: A Master's or higher degree in social work is required. LICENSES: - Licensure as a Licensed Certified Social Worker or Licensed Independent Clinical Social Worker by the Massachusetts Board of Registration in Social Work is required. - Based on assignment, travel may be required.
