The Florida Department of Health (DOH) is responsible for the health and safety of all citizens and visitors to the state. As a public health agency, the Florida Department of Health must monitor the health status of Floridians, diagnose, and investigate health problems, and mobilize local communities to address health-related issues. The Florida Department of Health develops policies and plans that support health goals, enforce laws and regulations that protect the health of all residents and visitors, and link people to needed health care services. The Florida Department of Health also provides specialized assistance to pregnant women, infants, and children with special health care needs, regulates health care practitioners, and provides medical disability determinations.

Title of Position:

OPS Social Work Services Program Consultant

Your Specific Responsibilities:

The incumbent in this position will serve in the Community Health Clinical Services Program and will support DOH-Hillsborough clients in the health-related social needs screening program. The incumbent will support assessment and navigation of clients, including some limited case management. The person in this position must also have a good working knowledge of computers and be comfortable communicating with clients. Bilingual skills in English/Spanish preferred. This position will require some local travel and may require limited weekend and evening hours.

  • Participate in program development, implementation, and quality improvement by making recommendations and follow through with implementation.

  • Evaluate and recommend necessary program protocols or procedures, in coordination with other DOH programs.

  • Conduct internal agency in-reach initiatives by attending meetings, networking, and participating applicable committees. Provide technical assistance within the agency to promote screening and navigation services.

  • Conduct community out-reach with local partner organizations by attending meetings, networking, and participating applicable committees. Gather resource information for the program and promote screening and navigation services.

  • Conduct client surveys and assessments by phone, web-based survey and in DOH clinic settings to measure health-related social needs among clients.

  • Implement a trauma-informed, resilience-oriented approach when interacting with clients.

  • Maintain an in-depth understanding of community resources and partner programs.

  • Input client data into existing data systems.

  • Summarize results in report form for monthly reporting requirements and other programmatic reporting needs.

  • Participate in and lead team meetings in rotation with other team members.

  • Precept social work interns when applicable.

  • Support other programs within the agency as needed.

  • Work before, during and after disasters if needed.

    Required Knowledge, Skills, and Abilities:

  • Knowledge of and ability to apply cultural competency and humility and understand how social and economic conditions can impact health.

  • Ability to provide support to clients using a trauma-informed, resilience-oriented approach.

  • Ability to conduct client interviews and case navigation.

  • Ability to maintain client confidentiality.

  • Ability to handle highly sensitive client interactions.

  • Ability to practice self-care to prevent compassion fatigue and burnout.

  • Ability to prepare reports.

  • Ability to plan, organize and coordinate work assignments.

  • Ability to communicate effectively, both orally and in written form.

  • Ability to understand and apply applicable rules, regulations, policies, and procedures.

  • Ability to establish and maintain effective working relationships with others.

  • Proficient in Microsoft Word, Excel, PowerPoint, Publisher, Outlook, and Teams.

    Minimum Qualifications:

  • Possess a bachelor’s degree in social work from a social work program accredited by or from an institution that is an active candidate for accreditation as a social work program by the Council on Social Work Education, as per Florida Statute 491.016.

  • At least one year experience with patient evaluation, intake referrals, case management or similar.

    Preferred

  • Bilingual in English / Spanish, written and spoken.
