JOB REQUIREMENTS: Position is anticipated to start 1-6-2025. The Coordinated Services Team (CST) Care Coordinator operates under WI Statute 46.56 in the provision of the Coordinated Services Team (CST) program. CST provides wraparound services to children currently engaged in services identified in the statute including, but not limited to mental health intervention, child protection, and juvenile justice. Receive and screen potential applicants to the CST program. Coordinate and facilitate the CST wraparound meetings. Assist families with developing their wraparound team including both their informal and formal support(s). Perform the strengths and needs assessment (CANS Assessment). Complete required data collection as outlined in state/program standards. Develop, implement and review the plan of care with the identified CST team according to timelines. Meet with families in their homes and other locations of their choosing as needed (at minimum on monthly basis). Documentation and distribution of CST meeting minutes to all stakeholders on the team within 2 business days. Weekly communication with any HHS unit sharing a case with a CST client. Facilitate the CST Coordinating Committee quarterly. Maintain client records. Attend all required CST training and meetings. Complete the state documentation for annual and 6-month review, and performance report for the CST program. Perform other duties as assigned. ***** OTHER EXPERIENCE AND QUALIFICATIONS: At minimum an Associate\'s Degree in Human Services or combination of training and experience equivalent to four years. Experience facilitating meetings required.
