The Care Manager Social Worker implements appropriate social service interventions and continuing care options. You will Interact with patients, families, physicians, and health care team colleagues using patience and tact.

A caregiver in this position works Monday throug h Friday 8:00am-4:30pm. No weekends. Will be required to work one major and one minor holiday per year.

Required qualifications for the ideal candidate include:

• Master's degree in Social Work

• One year experience in acute care social work or social work experience in the field

• Medical Clinical Knowledge

• Critical thinking skills, decisive judgment and the ability to work with minimal supervision.

• Ability to work in a stressful environment and take appropriate action.

• Familiar with local community resources
