About Pine Ridge
The facility sits on Hansen Care Park’s beautiful, wooded property with private and semi-private rooms, spacious shower and restroom facilities, beauty salon and barber shop services, walking paths, state of the art rehabilitation areas for those who need physical, occupational or speech therapy and cozy places to relax throughout the facility. We provide care for individuals who may require a variety of medical services including Short-term Rehabilitation, Long-term Care 24 Hour Skilled Nursing, Hospice and Respite Care, Restorative Care and Certified Wound Care.
Scope of Work
Responsible for, and will participate in, the assessment, planning, implementing, coordinating, communicating and finalizing the discharge of the patient/resident form the hospital setting or rehab and nursing center. Works with other members of the health care team both inpatient and outpatient, to advocate for and ensure appropriate aftercare and follow up.
Qualifications
Required Bachelor's Degree Social Work: Exceptions for persons without asocial work degree can be made for those who have a humanservices degree and carry a current social worklicense OR
Required Bachelor's Degree Social Work or a human services field including, but not limited to, Sociology, Gerontology, Special Education, Rehabilitation Counseling, and Psychology (Rehab and Nursing Centers only)
1 year of relevant experience Supervised social work experience in health care setting working directly with individuals. (Rehab and Nursing Centers only) Required
1 year of relevant experience skilled nursing home process and procedure Preferred
1 year of relevant experience mental health, substance abuse assessment and treatment Preferred
1 year of relevant experience community resources and discharge planning Preferred
3 years of relevant experience Hospital or Nursing Home Social work Preferred
LIC-Bachelor Social Worker (BSW) - STATE_MI State of Michigan Excludes Rehab and Nursing Centers Upon Hire required
