Gillette Children's Hospital - Phalen Outpatient Clinic Part Time - .6 FTE, 24 hours per week (Benefit Eligible) Work Schedule - 8-4:30 on weekdays, on-call shifts evenings and weekends
Assists patients and families in dealing with the impact of illness, disability and episodes of care in the hospital or a clinic. Promote health and wellness to the highest degree possible with regard to psychosocial functioning and strive to achieve maximum benefit from resources at Gillette and in the community. Modalities are geared to individuals, families, groups and consultation with other health care providers. The scope of services includes: psychosocial assessment, support and advocacy to facilitate positive coping, counseling, crisis intervention, case coordination, patient/family education, discharge planning and information and referral. Diagnostic evaluation and brief psychotherapy are also utilized in selected cases.
Compensation & Benefits
The hourly wage for this opportunity is $40,285/annually to $50,350/annually, with a median wage of $60,415/annually (adjusted to being a salary for a .6 FTE). Pay is dependent on several factors including relevant work experience, education, certification & licensure, and internal equity. Hourly pay is just one part of the compensation package for employees. Primary Job Responsibilities
Identifies presenting problem(s); and includes family concerns, strengths and expectations regarding the emotional, social, of health-related issues; and leads to recommendations and appropriate resources of support.
Integrates background information and assessment using DSM-V multi-axis diagnoses to drive appropriate interventions.
Demonstrate critical thinking and analyzing with complex psychosocial information.
Communicate assessment outcomes through verbal exchanges with patient, family, referral source and other staff involved in care.
Clarifies patient and family strengths, needs and goals and develop objectives to enhance patient functioning.
Design and implement therapeutic interventions appropriate to clinical problem and within own professional competencies.
Use a range of modalities to address identified needs, enhance strengths and support goals consistent with the mission of Gillette.
Modify modalities in consultation with patient and/or family and care team.
Communicates interventions through documented and verbal exchanges.
Interact with patients and families in a developmentally appropriate manner, recognizing the centrality of family or caregiver in care.
Show sensitivity and respect for diverse cultural groups.
Demonstrates ability to work constructively with complex patient and families.
Integrates patient and family focused care in social work practice by applying principles of patient/family-centered care.
Researches, mobilizes and coordinates resources and the delivery of internal and external services that enable the patient and family to achieve identified goals.
Actively participates in discipline and interdisciplinary meetings
Collaborate with other Gillette staff, the patient’s school and community resources
Qualifications
Required
MSW
Licensed Independent Clinical Social Worker by the MN Board of Social Work
Current CPR certification optional, if desired can certify and recertify through Gillette
Basic computer skills
Knowledge, Skills and Abilities
Ability to work effectively on interdisciplinary teams
Interviewing and counseling skills and the ability to conduct psychosocial assessments and interventions in a condensed time frame
Ability to establish and maintain therapeutic rapport
Knowledge of psychosocial development across the life span, emotional and social impacts of disability and illness, community resources related to pediatric health care and disability disorders, mandated reporting obligations and laws and financial systems and services related to payer sources.
Preferred
- Prior experience in hospital or health care social work is strongly preferred
