Responsibilities Job Description
The Lead Service Coordinator role is a working manager position. In addition to the traditional duties of a Service Coordinator, you'll be the main point of contact for questions for your team regarding procedures. This position will also assist in training and education, supervising compliance and comprehension of procedures, perform a variety of clerical functions and coordinate daily deliveries for the FHME drivers.
This role will help to ensure a safe and secure environment to employees and patients, while maintaining great patient care, under all relevant laws, regulations and policies.
Responsible for on-site training and monitoring of compliance with procedures
Monitors new employee progress at specified intervals during first months of employment. Will provide support, answer questions and follow up initial training
Visit expert staff twice yearly to answer questions and get feedback from staff.
Follow-up training on new procedures as introduced by clerical supervisor to assess understanding, compliance to reduce variation among locations and staff
Develops and coordinates care protocols and DME set-ups for Home Medical Equipment patients.
Develops, implements, and manages inventory control process in collaboration with vendors and FV support services to ensure care plans are implemented appropriately.
Ensures inventory costs are in line with departmental budget
Configure DME equipment and fit for appropriate accessories
Coordinates equipment data management and analysis
Develops and implements processes to support patient care in alignment with DME compliance data
Collaborates with care team to ensure care plans are supported with timely and accurate information
Provides timely and accurate compliance information to providers
Facilitates effective claim processing with DME aspects of care plan
Enable accurate claim process by aligning billing processes with insurance requirements
Partner and/or lead in quality improvement initiatives and community education events
Completes weekly retail deposits following Fairview’s cash handling procedures
Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served
Creating individualized plans of care to meet patient needs, partners with patient care giver in care/decision making, and providing patient education based on assessment of learning needs of patient/care giver

Qualifications
Required
Education
- Associate Degree in Healthcare or equivalent work experience
Experience
- 1 year working in a clinic setting
License/Certification/Registration
- BCLS, or required to obtain within 1 month from hire date
Preferred
Education
- Bachelor’s Degree in health care related field
Experience
2+ years in a direct patient care setting
6 months experience fitting DME/respiratory equipment
