The incumbent provides integrated population health expertise and supports policy development with a distinct focus on conducting community outreach, following the vision set by DHS Medicaid Medical Director.
Responsibilities include:
Develop and execute community outreach efforts to undertake broad reforms in ensuring equitable access to quality integrated health care for people of MN.
Assess health care needs, utilization, service delivery, costs, and outcomes; health disparities; and other epidemiological or demographic studies as needed by the administration in concert with our internal and external partners.
Provide leadership, direction and coordination of all phases of grants and contracts process across multiple program lines or all aspects of a specific program’s operation.
Minimum Qualifications

Three (3) years professional experience demonstrating expertise with government grants and contract competencies.
Experience must clearly demonstrate:
Strong understanding of core contract and grant management standards and practices
Experience negotiating contracts terms to a wide variety of contractors and/or grantees
Data research skills sufficient to identify best practices, abnormalities and track measurable administrative duties;
Program and project management skills that also includes tracking grant budgets and presenting program efforts.
*Bachelor’s Degree in Social Work, Psychology, Public Health, or another related fields may substitute for one (1) year of work experience.
Master’s Degree in Social Work, Psychology, Public Health, or another related fields may substitute for two (2) years of work experience.**
Preferred Qualifications
Certified by the National Grant Management Association (NGMA) as a Certified Grant Management Specialist (CGMS)
Variety of experiences working effectively with others from different backgrounds and cultures
Knowledge of Medicaid and Minnesota’s Health Care Programs and/or delivery systems
Engagement, outreach and facilitation skills to negotiate a variety of interests to reach consensus
Knowledgeofrolesandrelationshipsofcounty,tribal,stateandfederalgovernments
Strong human relations and communication skills; communicate clearly and professionally both verbally and in writing; respond openly to requests and concerns.
