The Life Manager is responsible to engage eligible members in the Lifespark Experience through proactive outreach and the implementation of Lifespark's proven process. He/she works one-on-one with members to discover what is most important to the member and develop a customized Life Plan that addresses all 7 elements of well-being. She/he manages the Life Plan, providing proactive guidance and connecting to essential 7 element services to deliver results. Acts within the full capacity of her/his licensure. Qualifications Education: * Registered Nurse, BSN preferred * Prefer advanced certification as a Care Manager, Holistic Nursing, Geriatric CM, and/or Gerontology Experience: * 2 years' RN experience in community-based services (assisted living, home care, hospice, public health) * Experience working with seniors preferred * Experience in customer service/direct member contact field preferred * Experience in multiple settings and situations * Experience in long term goal setting and pro-active intervention Knowledge, Skills and Abilities: * Aging process and related care issues * Advanced care planning * Assessment and person-centered care planning * Caregiver and member education * Community resources * Competence and comfort working with complex family dynamics * Creativity and ability to solve problems * Ability to create trusted relationships with members/families * Proficient with technology and relevant applications: Microsoft Office, Smart Phone, Calendaring, Clinical Software platforms (EMR), Wi-Fi, Printers, and Internet Browsing * Strong written and oral communication skills * Time and territory management It has been and will continue to be our policy to provide equal employment opportunities to all employees and applicants without regard to age, race, creed, color, disability, marital status, sex, gender identity, national origin, ancestry, sexual orientation, arrest record, conviction record, military service, use or nonuse of lawful products off the Employer's premises during nonworking hours, or declining to attend a meeting or to participate in any communication about religious matters or political matters, or any other status protected by federal, state, or local law. #populationhealth
