JOIN TEAM TRILOGY Summary Supports the day-to-day functions of the Legacy Lane with passion for providing care and education of dementia while adhering to current federal, state and local standards, guidelines and regulations that govern long-term care facilities. Duties and Responsibilities 1. Coordinates, with the direction of the Executive Director, the leadership team, responsibility and accountability of general daily operations of the Legacy Lane programs of the health campus 2. Manages all aspects, including monthly team education, of the Best Friends Forever (BFF) Program 3. Ensures Daily Rhythms activity programming is occurring per company standards 4. Maintains Department Leader Rounding binder and partners with Executive Director to ensure follow-up 5. Assists with maintaining appropriate resident population as outlined in the company's memory care move in/move out criteria 6. When possible, participates in all memory care Resident First Meetings/Service Plans and assists to guide resident family members through the campus' care continuum 7. Develops and maintains relationships with resident family members to ensure excellent customer service 8. Provides and reviews Family Orientation Guide with new family members 9. Maintains open communication with campus staffing coordinator to ensure consistent dementia staffing and support on the Legacy 10. Interviews potential Legacy team members to ensure candidates possess basic skills needed to work with residents with dementia 11. Maintains a dementia-safe environment through daily resident room and Lane rounds, management of noise and other environmental stimuli throughout the Lane 12. Oversees and regularly assists with meal service adhering to the company's Family Style Dining Standards 13. Leads efforts to educate campus staff and the community in dementia-related topics and acts as a campus "Dementia Champion" 14. Partners with marketing to ensure First Impression Standards are in place and ensures tour readiness on the Lane. WHAT WE'RE LOOKING FOR Knowledge of: Operational services terminology, principles and practices of dementia care Person-centered care philosophies and principles Medicare and Medicaid guidelines Health care principles, practices, regulations, concepts and procedures as applied to the care of patients in a skilled or assisted living facility Compliance with external regulatory agencies and organizations for all areas of responsibility Skills in: Ongoing development of a servant leadership culture that strives to achieve the highest ethical standards Organizing multiple tasks simultaneously and adhering to goals and/or deadlines Organization with strong attention to detail Validation approach in communicating with those living with a dementia diagnosis Creative and holistic approaches to the care of behavioral complexities of those living with dementia Effective group activity facilitationAssists with assessing resident psychosocial needs and developing plan of care based on those needs Ability to: Communicate effectively, both orally and in writing and identify and recommend solutions to problems Operate standard office equipment and use required software applications to produce correspondence, electronic communication and maintain spreadsheets and databases Maintain confidentiality and professional discretion Deal tactfully with personnel, campus department leaders, residents, family members, visitors, government agencies/personnel, vendors, and the general public Effectively present information and respond to questions from groups of managers, and customers Review and analyze report information, while using critical thinking skills to evaluate data for regulatory and standards of compliance Organize, prioritize and track multiple tasks with strong attention to efficien
