Baystate Health Community Paramedic in Springfield, Massachusetts

Baystate Health, western Massachusetts’s premier healthcare provider, is looking for an experienced Community Paramedic.

The Community Paramedic assists with facilitating patient to provider communication and creating inclusive and accessible healthcare and will deliver an extension of primary care in the community through a robust physician led model. This model will decrease and prevent further high ED and acute inpatient utilization. The Community Paramedic is integral with collaborating with specialty care for high-risk patients and providing disease management in the community to support the patient and specialty providers.

Qualifications:

  • Bachelors Degree

  • Paramedic Certification

  • BLS CPR certified

  • Minimum of 5 years of EMS experience with at least 3 years as a paramedic.

  • Valid Drivers License

  • Experience with training, CQI, marketing and/or public communication experience and proficiency.

Job Responsibilities:

Administers basic and advanced assessment and care to sick and injured persons in the out-of-hospital setting under the direction of emergency medicine and community health center physician(s). Supports existing health services; provides integrated health services in partnership with other health professionals; extends access to health services delivery in underserved and general populations, including primary care, public health, disease management, prevention and wellness, mental health, and dental health; and performs other duties as required.

  • Examines, screens, treats, and coordinates health services for patients.

  • Conducts post-hospital release follow-up care including, but not limited to, monitoring medication, dressing changes, and checking vital signs.

  • Observes, records, and reports to the physician, patient’s conditions and reaction to medications, treatments, and significant incidents.

  • Conducts patient education, including diabetes prevention/treatment, hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), falls assessment, injury evaluation, geriatric frailty visits, and nutrition.

  • Administers patient care consistent with clinical guidelines and provider orders; Coordinates appointments and follow-up with physicians, hospital staff, and community health center staff.

  • Develops and completes appropriate reports and templates for the mobile care program and attends meetings as requested and available.

  • Operates a Baystate Health vehicle in accordance with Baystate Health policies.
