Position Summary
Under the supervision of the Licensed Nurse, the NAC/NAR assumes responsibility and accountability for a group of residents/patients for a shift of duty. The NAC/NAR is to assist the residents in maintaining the highest quality of life possible within the functional limitations of each resident The NAC/NAR shall assist the residents with basic ADL care and promote the psychosocial well being of each resident. The NAC/NAR shall follow the care plan to meet the needs of each individual resident. Follows all facility policies and procedures. Performs duties as defined by the State Nurse Practice Act.
Job Duties
Gives/receives report upon reporting in and ending shift duty hours.
Reads and has basic understanding of behavior management programs.
Must be respectful to residents, visitors, co-workers and interact appropriately for this setting. Residents and proper care are the number one priority for this facility.
Must possess a positive attitude and understand the importance of teamwork.
Observes and provides a comfortable, clean, safe environment at all times.
Reports any suspicions or allegations of abuse or neglect to the LN right away.
Answers call lights whenever and wherever needed.
Handles on-call responsibilities as required.
Demonstrates proper procedures for Admission, Transfer and Discharge.
Demonstrate the ability to locate and follow the Nursing Plan of Care, Nurse Aide Flow sheet, and be able to document according to established procedures.
Demonstrates the ability to locate emergency equipment.
Complies with laws and regulations applicable to position and acts in accordance with North Central Care Center policies.
Can identify dietary restrictions for special diets such as: NCS, NAS, mechanically altered and thickened fluids. Follows protocol for thickening fluids.
Demonstrates proper feeding technique utilizing safe swallowing guidelines, assist and cueing as resident requires.
Always sits next to a resident while assisting with feeding procedures.
Encourages fluid intake and reports to LN inadequate fluid intake.
Demonstrates appropriate bed making procedures, either occupied or unoccupied.
Demonstrates proper procedures for bathing residents (Bed bath, partial and complete, tub bath, shower, towel bath, perineal care, and whirlpool bath).
Demonstrates proper procedure for dressing residents, bedfast or ambulatory.
Demonstrates shaving a resident, male or female.
Demonstrates appropriate hair care for residents.
Demonstrates appropriate nail care and hand washing of residents. Nails are kept neatly trimmed and hands are cleaned before and after meals. Exception: Diabetic residents or residents on Coumadin must have nail care performed by a licensed nurse.
Demonstrates appropriate oral and dental care, routine care for resident's mouth, denture care, natural teeth, edentulous residents, and mouth care for the unconscious patient and proper use of lemon-glycerin swabs or sponge type swabs.
Demonstrates appropriate procedures for routine skin care and use of lotion.
Demonstrates proper technique in use of items to prevent skin breakdown, sheep-skin, booties, elbow/heel protectors, flotation mattress/chair cushions and proper application of incontinent briefs, incontinent pads and proper use of barrier creams.
Reports any new instances of skin breakdown to LN right away.
Demonstrates appropriate procedures for care of external catheter, indwelling catheter, proper positioning of drainage tubing, proper position and covering of drainage bag for dignity purposes. Follows correct procedures for emptying Foley bag.
