Job Overview:
RWJBarnabas Health is looking to add a qualified full-time Pharmacist Sen Transitions of Care in Somerville, NJ. The Pharmacist will be responsible for reviewing and optimizing medication therapy, in collaboration with other healthcare practitioners, for patients transitioning between acute care and outpatient settings
Qualifications:
Required:
Doctor of Pharmacy degree from an ACPE-accredited school of pharmacy
Completion of a PGY1 Pharmacy Residency in general practice
Preferred:
- Completion of PGY2 Pharmacy Residency training in Internal Medicine Pharmacy, Pharmacotherapy, or Ambulatory Care Pharmacy
Certifications and Licenses Required:
- Must be a NJ Licensed Pharmacist
Scheduling Requirements
Day Shift, 40 hours/week
Full Time Schedule
Essential Functions:
Provides individualized patient education/evaluation regarding drug therapy as requested or identified by need
Evaluates appropriate laboratory tests necessary to monitor and support the patient s drug therapy regimen
The clinical specialist will actively promote medication safety through the identification and analysis of adverse drug events and medication incidents; develop and implement evidence-based and/or cost-savings initiatives; and provide educational expertise and support to patients/families and other healthcare disciplines within the hospital
Educates healthcare professionals to ensure the safe and effective use of medications in systems and/or populations
Ensures that appropriate standards of practice follow corporate compliance guidelines, acts in an ethical and legal manner, and assures that all actions are in the best interest of the hospital
