As part of the Social Determinants of Health (SDOH) assist the Yuma County community members including\ Migrant Seasonal Farmworkers (MSFWs) navigate social and community services related to socioeconomic,\ education, employment, social support networks, and access to health care services by coordinating\ comprehensive outreach efforts and providing chronic disease self-management educational services.\ Promotion of health programs through coordinated comprehensive and effective outreach efforts. To develop and\ implement educational/outreach programs to effectively meet the health care needs of Wellton, Yuma, Somerton\ and San Luis areas including migrant and seasonal farm workers. Schedule and manage appointments for patients\ with chronic disease. Document patient education and chronic disease related clinical data.
