We are looking for an Onsite Health Coach to join our team and deliver high-quality coaching sessions to our clients at their workplace. You will be responsible for assessing their health needs, creating personalized action plans, and providing ongoing support and guidance to help them achieve their goals.

This Health Coach Consultant RN role is an onsite position based in Scranton, South Carolina and is NOT a remote position. Six to eight weeks of training to be completed remotely and upon completion will be required to sit onsite in Scranton, South Carolina . This position will be supported onsite by both Aetna and the client health promotion team.

Normal Working Hours: Employee will typically work Monday through Friday 8:00am to 4:30pm EST with some flexibility required. The employee will work 4 days per week in Scranton, South Carolina and then travel 1 day per week to locations throughout South Carolina and Northeast Georgia. This schedule may adjust per the needs of the plan sponsor.

There are no weekends or holidays required .

The Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation, and evaluation, to engage, educate, promote, and influence member's decisions related to achieving and maintaining optimal health status. The Health Coach Consultant also demonstrates commitment to continuing professional development, has an energetic and compassionate personality, and is willing to teach, learn and be open to new and innovative ways of delivering preventive and intervention services. You will demonstrate insight into human behavior and can communicate, interact, and build rapport with individuals at varying education levels. You will also prioritize and organize your own work to meet deadlines. The Health Coach Consultant will preferably have a history of success working in less structured, self-directed environments.

Primary Responsibilities:

  • Performs strategic outreach to identified high risk members, including in-person onsite activities and opportunities for creative engagement

  • Meets one-on-one to provide motivation, monitor progress, and assist employees in developing personal and measurable health goals and plans

  • Reduces gaps in member health service by providing referrals

  • Supports the Client Company's Culture of Health workplace initiatives

  • Promotes strategies for lifelong wellness in the healthy member population through education and accountability for prevention and lifestyle interventions focusing on nutrition, physical activity, weight management, stress management

  • Assesses members using clinical tools and information/data review, conducts comprehensive evaluation of member's needs and benefit plan eligibility for available integrated internal and external programs/services.

  • Utilizes assessment techniques to determine member's level of health literacy, technology capabilities, and/or readiness to change.

  • Applies and/or interprets applicable criteria and guidelines, health/wellness management plans, policies, procedures, regulatory standards while assessing benefits and/or member's needs to enable appropriate utilization of services and/or administration and integration with available internal/external programs.

  • Utilizes influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health.

  • Provides coaching, information, and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices.

  • Interprets and utilizes clinical guidelines/criteria to positively impact members health Provides up-to-date healthcare information to help facilitate the member’s understanding of his/her health status.

  • Helps member actively and knowledgably participate with their provider in healthcare decision-making.

  • Monitors established plans of care

Required Qualifications

  • 3+ years of clinical experience as an RN in acute care setting (med/surg; ER)

  • Must have an active, current, and unrestricted compact RN licensure in the state of South Carolina

  • Minimum of 1 year of previous coaching experience

    • Ability to commute to/from Scranton, South Carolina for onsite work each shift; Monday through Friday 8am to 4:30pm EST.
  • Must be willing and able to work outside of standard hours Monday through Friday due to business needs up to once a week

    • Must live within 45 minutes of Scranton, South Carolina

    • Ability to travel a minimum of 1 day per week throughout the state of South Carolina and to Northeast Georgia per the need of the company

Preferred Qualifications

  • Direct education and coaching experience with individuals for disease prevention, health education, or behavior change preferably in a corporate setting

  • Managed care experience

  • Experience in multiple areas of nursing in an acute setting

  • Motivational Interviewing experience

  • A certification as a health coach from a recognized organization

  • Excellent communication, interpersonal, and motivational skills

  • Able to work independently and as part of a team

  • A passion for helping others improve their health and well-being

  • Proficient in Microsoft Office and online platforms

Education

  • Associate degree in nursing required

  • BSN or a bachelor’s degree in health education or health promotion preferred

Pay Range

The typical pay range for this role is:

$54,095.60 - $116,800.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
